I agree that, related to the access of information submitted and entrusted to me by the Cagayan State University, I will:

1. Keep all information relevant to the ten (10) areas of accreditation provided to me for the program/programs being visited via this Online Accreditation System (OAS) .

2. NOT to use any of the information entrusted to me to advance the interest of the SUC where I am currently employed and /or for my own personal gains.

3. Disclose this information only to my accreditation team partner/s who also signed a non-disclosure agreement or to a person or persons who have been given the authority by the SUC whether verbal or written to receive this information; and,

4. Disclose the final online accreditation report solely to the Accrediting Agency of Chartered Colleges and Universities of the Philippines (AACCUP) to be deliberated upon by the Board of Trustees for the grant of the appropriate level of accreditation sought; and, the National Network of Quality Assurance Agencies.
